Event
Number: Some regions identify each activity with an
event number. If you do, enter it in this field.
Title*: Title of the workshop. (This is a required field)
Description: A
description of the workshop no more than 500 characters.
Date*: Format: mm/dd/yyyy. If the
workshop will be held on multiple days, you enter the first date in field one
and the second in field two – to add additional dates, go to the next field:
Other Dates: (This
is a required field)
Other Dates: Format:
mm/dd/yyyy, mm/dd/yyyy, mm/dd/yyyy.
Enter multiple dates separated by a comma.
Time: Format: hh:mm am/pm. The time the workshop
will start – must also enter am or pm.
There should be one space between the hh:mm and am/pm.
Provider*: You will not be able to change this
field. If the provider field is
different than yours, please contact us. (This is a
required field)
Other Providers: You may select from other available providers from the list on the left if you have partnered with them in providing the workshop. When you do this, the workshop will be displayed not only in your region but in the region that you have partnered with. If you have partnered with a whole area, you may check the appropriate area. If you want the workshop to be displayed statewide in each regional office listing, you would check the statewide box.
Category*: You must categorize your workshop into one of
the existing categories. (This is a required field)
Alternate Category: If the workshop meets the 20% special education requirement, you would select special education from the drop down box.
Area*: You will not be able to change this
field. If it is different than the one
you are in, please contact us. (This is a required
field)
Status*: You must select from Open, Full, Canceled, or
Closed. This field can be changed as the
status of each of your workshops change. (This is a
required field)
Meals: If you will be providing a meal, you would
check the box.
Registration Deadline*: Format:
mm/dd/yyyy. Deadline when an individual
can last register for the workshop.
Workshop Address: The
address of the location for the workshop. This should include street address, city,
and state.
Target Audience: Any combination K-12th, K-6th, 6th- 8th, etc, or Administrators, Counselors, Social Workers, etc.
Presenter: Enter the name of the
presenter or presenter’s conduction the workshop.
Enrollment Limits: If the workshop has an
enrollment limit, enter it in this field.
CPDUs: Enter the number of CPDUs being given (##.#).
Administrators’
Academy Credit: Enter the number of CPDUs being given (##.#)
In Region Cost: Enter the cost of the workshop
($00).
Out of Region Cost: Enter the cost for those who might attend from another region ($00).
Co-op Member Cost: Enter the cost for co-op member
if you have a co-op ($00).
Member Cost: Enter the cost for member
if you have a membership ($00).
Non-Member Cost: Enter the cost for
non-member
if you have a membership ($00).
Special Funding Cost: Enter the
cost for special funding cost if it is funded differently ($00).
Cost Per Semester Hr: Enter the cost per semester
hour of credit ($00).
Graduate Credit: Enter the number of semester
hours of credit being offered.
University: Enter the name of the university offering the course.
Course Number:
Enter
the number of the course being offered by the university.

email address of the person who can answer any questions a person might have about
a workshop.
Registration Information: Enter the
URL of your website that would allow an individual to go to your website and
either register electronically or print a form to register. The URL must have the http://www
entered before your local address to make the link work properly. Example: http://www.whitesideroe.org/InService/images/PDF/reg_form.pdf
Total Attending: After the workshop has been completed, please enter the number of participants attending the workshop.
